Develop a Transition Plan Based on Input from IEP Team
The Transition Plan is generated from the results of the Transition Survey. It summarizes the student s Postsecondary Goals , as well as the student s transition strengths and needs. The plan shows the method that will be used to meet each need and who is responsible for helping the student meet each need.
