Targeting Transition Materials
The Targeting Transition program includes a comprehensive set of materials that help identify transition needs, write measurable transition goals, teach transition skills, and track transition progress over multiple years.

The program includes four transition rubrics manuals for four different types of student. Each manual contains a survey to assess student transition needs, transition rubrics for each skill that pinpoint the expectations of the skill, and a multiple year tracking form.

The program includes activities that teach the transition skills in two of the four manuals, including activities for skills in:
- Rubrics for Transition I: for Higher-Functioning (independent) Students
- Rubrics for Transition II: for Students with Moderate Disabilities (somewhat dependent).
The activities include lessons, interviews, practice, evaluations, plans to improve, and a variety of other tasks.

The program includes a trainer’s manual that provides the tools for training staff members in the use of the program. The trainer’s manual includes program forms in digital format, a hard-copy explanation of training procedures, and a series of PowerPoint presentations that can be used to train the staff in one or several sessions.
