The 43 transition skills for students with severe disabilities are organized into tools that enhance the transition process, including: (1) a transition survey—used to assess transition needs, (2) a transition rubric for each skill—used to write transition goals, teach transition skills, and collect transition data, and (3) a progress report—used to track multiple-year transition progress.
