Transition Skills for Students with Severe Disabilities

Rubrics for Transition IV: for Students with Severe Disabilities is designed for individuals with severe and/or multiple disabilities who will live dependent and supported adult lives. The manual contain 43 transition skills for students with severe disabilities in the areas of Workplace Skills and Attitudes, Responsibility, Interacting with Others, Technology Skills, Basic Academic Skills, Habits of Wellness, and Planning for Success.

The 43 transition skills for students with severe disabilities are organized into tools that enhance the transition process, including: (1) a transition survey—used to assess transition needs, (2) a transition rubric for each skill—used to write transition goals, teach transition skills, and collect transition data, and (3) a progress report—used to track multiple-year transition progress.
